Job summary

FOX Rehabilitation in Chattanooga, TN is seeking a licensed Speech Language Pathologist to provide one-on-one care in home and senior living settings. You will evaluate, plan, and deliver evidence-based therapy to improve communication, swallowing, and quality of life for older adults.

You will document in our EMR, collaborate with an interdisciplinary team, and have the flexibility to tailor your schedule. Telehealth options may be incorporated to extend reach and continuity of care.

Qualifications

• Must hold a valid Speech Language Pathology license in the practicing state or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification and good standing with licensure.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document in FOX’s EMR system in line with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
