Job summary

FOX Rehabilitation in Marlton, NJ seeks licensed Physical Therapists to deliver one-on-one care in homes and senior living communities, part of the Geriatric House Calls model. You'll conduct evaluations, develop plans of care, and provide evidence-based therapy to improve mobility and independence.

Clinicians document in an EMR, collaborate with PT/OT/SLP, and educate patients and staff to promote aging in place. Telehealth options offer flexibility and broader patient access.

Qualifications

• Active Physical Therapy license in state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
