Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one‑on‑one care in homes and senior living communities in Waukegan, IL. You will evaluate, plan, and deliver evidence‑based therapy to older adults, helping them regain mobility and independence. Telehealth options may be incorporated to extend access and continuity of care.

The role offers autonomy with a flexible schedule, team collaboration, and opportunities for professional growth within a clinician‑driven practice.

Qualifications

• Valid Physical Therapy license in the state(s) of practice
• Degree from an accredited Physical Therapy program
• Current CPR certification

Responsibilities

• Provide one‑on‑one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence‑based treatment
• Deliver therapy sessions to promote mobility and independence
• Document in FOX’s EMR per regulatory standards
• Collaborate with PT, OT, SLP, and Fitness Specialists
• Educate patients and caregivers to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
