Job summary

FOX Rehabilitation is seeking a Physical Therapist for Bloomington, IN to provide one-on-one in-home and senior living care. Responsibilities include taking evaluations, creating tailored care plans, and delivering evidence-based therapy to improve mobility and independence.

The role supports telehealth options, interdisciplinary collaboration, and education for patients, families, and staff. A sign-on bonus is available for qualified clinicians.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home and community settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy to support wellness, mobility and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
