Job summary

FOX Rehabilitation in Chatham, NJ is seeking licensed Physical Therapists to deliver one-on-one geriatric care in private residences and senior living communities as part of the Geriatric House Calls model.

You’ll build a flexible schedule, utilize telehealth when appropriate, and collaborate with an interdisciplinary team to improve mobility and independence for older adults we serve.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
