Job summary

FOX Rehabilitation in Bridgewater, MA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes or Senior Living Communities, supporting mobility, independence, and quality of life through our Geriatric House Calls model.

Clinicians may incorporate telehealth and partner with an interdisciplinary team; you’ll create and manage your schedule, build lasting patient relationships, and enhance care across the community we serve.

Qualifications

• Licensed Physical Therapist in MA or eligible to obtain license.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
