Job summary

FOX Rehabilitation is seeking licensed Physical Therapists for in-home and senior living settings in Ottumwa, IA. Deliver one-on-one therapy to older adults in private residences or communities, with emphasis on mobility, independence, and quality of life through the Geriatric House Calls model.

Clinicians enjoy autonomy to create and manage their schedules, collaborate with PT/OT/SLP teams, and may incorporate telehealth to extend care and access for more patients.

Qualifications

• Licensed Physical Therapist in the state of practice or eligible to obtain license.
• DPT or equivalent degree from an accredited program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and function.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging in place.
• Provide telehealth/virtual care when appropriate.
