FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one in-home and senior living care in Grinnell, IA. You will evaluate patients, develop individualized plans, and deliver evidence-based treatments to improve mobility and independence.

Highlights include autonomy in scheduling, potential telehealth integration, and collaboration with an interdisciplinary team to enhance patient outcomes and aging-in-place support.

Qualificações

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• EMR proficiency and basic computing skills.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and safety.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
