Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to join our in-home care team in Ottumwa, IA. You’ll deliver one-on-one PT services in private residences or senior living communities, focusing on mobility, independence, and aging in place.

You’ll design individualized plans, document in FOX’s EMR, and collaborate with an interdisciplinary team, with the flexibility to manage your own schedule and add telehealth when appropriate.

Qualifications

• Licensed Physical Therapist in Iowa or eligible to obtain license.
• Degree from an accredited Physical Therapy program (DPT or equivalent).
• Current CPR certification and ability to maintain licensure in good standing.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and functional outcomes.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
