Job summary

FOX Rehabilitation in Grundy Center, IA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in private residences or Senior Living Communities. You’ll help older adults improve mobility, independence, and quality of life through our Geriatric House Calls model.

This role offers autonomy to create and manage your own schedule, opportunities to incorporate telehealth, and a supportive, interdisciplinary team focused on meaningful patient outcomes.

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver sessions that support mobility, independence, and functional goals.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
