Job summary

FOX Rehabilitation in New Jersey is seeking licensed Physical Therapists to provide one-on-one care in patients' homes and senior living communities. This flexible role supports autonomy, scheduling, and meaningful patient outcomes, with opportunities for in-home, clinic, and telehealth delivery.

Responsibilities include evaluations, plans of care, and collaborative care with PTs, OTs, SLPs, and Fitness Specialists to improve mobility and independence for older adults.

Qualifications

• Active Physical Therapy license in the practicing state or eligibility to obtain one.
• Current CPR certification.
• Ability to document and use EMR systems effectively.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that improve mobility, independence, and quality of life.
• Document in the EMR system in accordance with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
