Qualifications

• Active California Physical Therapist license.
• Experience with older adults or home health preferred.
• Strong communication and motivational skills.
• Ability to design adaptable therapy plans based on patient progress.
• 1+ year PT clinical experience; geriatric population preferred.

Responsibilities

• Assess patients to identify physical therapy needs.
• Develop customized home health treatment plans focused on mobility and balance.
• Provide in‑home therapeutic exercises, stretching, and fall prevention.
• Educate patients and caregivers on safe mobility and home exercises.
• Track progress and adjust treatment plans to achieve outcomes.
