Job summary

All Seniors Foundation is seeking a Home Health Physical Therapist to design and implement personalized therapy programs for seniors, improving strength, balance, and mobility in the home setting.

You will assess, plan, and adapt treatments, educate patients and caregivers, and document progress to support independence and quality of life for older adults in Los Angeles County.

Qualifications

• Valid Physical Therapist license in California.
• Experience with older adults or in home health preferred.
• Excellent communication, patience, and motivational skills.
• Ability to tailor therapy plans based on patient progress.

Responsibilities

• Conduct thorough patient assessments to identify PT needs.
• Develop customized treatment plans focused on mobility, pain management, and fall prevention.

Role Overview
