Job summary

Lifespan Therapy seeks a Home Health Physical Therapist to provide patient-centered care across California through our app-based network. Flexibility in geography within the Los Angeles area is offered, with visits in neighborhoods such as Echo Park, Hollywood, Silverlake, and beyond.

Qualifications

• California PTA license is required.
• Driver's license and auto insurance are required.
• Spanish language is a major asset but not required.

Responsibilities

• Conduct in-home evaluations and develop individualized plans of care for geriatric patients.
• Focus on functional strengthening, gait training, balance, and fall risk reduction to improve mobility.
• Schedule and conduct home visits based on availability and patient needs.
