Job summary

FOX Rehabilitation is expanding in Burlington, MA, seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes or Senior Living Communities, using our Geriatric House Calls™ model.

You’ll evaluate, plan individualized care, deliver evidence-based treatment, and document in FOX’s EMR while collaborating with PTs, OTs, SLPs, and Fitness Specialists. Telehealth options may be incorporated to increase access and flexibility.

Qualifications

• Must be a licensed Physical Therapist in MA.
• Experience delivering one-on-one PT in home, private residence, or senior living settings preferred.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
