FOX Rehabilitation in Burlington, MA is seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities, using our Geriatric House Calls model and telehealth when appropriate.

You will evaluate patients, develop plans of care, document in the EMR, and collaborate with PT/OT/SLP teams while educating families to promote safety and aging in place.

Qualificações

• Valid Physical Therapy license in the state of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilidades

• Provide one-on-one PT to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access
