Job summary

FOX Rehabilitation is expanding in Burlington, MA and seeks licensed Physical Therapists to provide one-on-one geriatric care in homes and Senior Living Communities. You’ll evaluate, plan, and deliver evidence-based therapy, with flexibility to manage your own schedule and incorporate telehealth as needed.

You will collaborate with an interdisciplinary team, educate patients and caregivers, and support aging in place while enjoying autonomy and meaningful outcomes.

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligible to obtain license.
• Degree from an accredited Physical Therapy program (DPT preferred).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and quality of life.
• Document in FOX’s EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary PT/OT/SLP and Fitness Specialists team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
