Job summary

Visiting Physician Services (VPS), part of the Parker Advanced Care Institute at VNA Health Group, seeks a Physician Assistant to deliver high-quality, in-home primary care to older adults who are homebound or medically complex.

As a key member of an interdisciplinary team, you will assess patients, develop care plans, monitor responses, and coordinate with families and community health partners to keep patients safe at home. NJ PA licensure required with prescriptive authority.

Qualifications

• Completion of Physician Assistant accredited program.
• NJ licensure with prescriptive authority; maintain a CDS and DEA.
• National certification as a PA with AAPA.
• Experience providing care to Adult or Geriatric patients.
• 2+ years of Physician Assistant experience and/or significant experience as a registered nurse.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assesses, diagnoses and develops appropriate plan of care to treat homebound patients.
• Monitors the patient’s condition and responsiveness to the treatment provided.
• Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations.
• Participates in organizational wide QAPI program and attends agency sponsored in-service programs.
• Adheres to agency infection control and safety policies.
• Adheres to the organization’s policy in regards to absenteeism and appearance.
