Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to provide in-home primary care to frail geriatric patients throughout the MetroWest region, with Worcester as a base. This autonomous, patient-focused role offers panel ownership, strong clinical support, and meaningful impact on each patient’s life. Expect to see about 6–8 visits per day.

Qualifications

• NP or PA with Massachusetts licensure (or ability to obtain).
• National Board Certification
• BLS certification required.

Responsibilities

• Provide in-home primary care to frail geriatric patients in the MetroWest region.
• Own and manage your patient panel with collaborative team support.
• Autonomous practice with strong clinical support.
• See approximately 6–8 visits per day.
