• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Part-time, day position with no weekends or evenings.
• Chronic condition management of a geriatric panel as the PCP, 3 days per week (Mon, Tue, Fri).
• Provide patient-centered home-based primary care under a value-based care model.
