Job summary
HarmonyCares is seeking a Nurse Practitioner to deliver high-quality in-home primary care to high-acuity patients in the Kent, Federal Way, Auburn and Renton areas. This role supports a value-based care model with a focus on patient-centered outcomes.
Expect to work in-home Monday, Tuesday and Friday, with a collaborative team and office resources including CME stipends and standard benefits.
Qualifications
- Experience delivering primary care in home-based settings.
- Geriatrics or hospice care experience is preferred.
- Active NP license in WA or ability to obtain quickly.
Responsibilities
- Provide primary care to high acuity patients in their home.
- Collaborate with the care team to coordinate in-home visits in Kent, Federal Way, Auburn and Renton.
