Job summary

HarmonyCares is seeking an Advance Practice Provider (NP/PA) dedicated to managing chronic conditions for high-risk geriatric patients in a home-based primary care setting. This onsite, salaried role operates Monday–Friday with patient-centered care at the core.

Ideal candidates hold NP/PA licensure in Michigan, DEA eligibility, CPR/BLS, and board certification; geriatrics experience is preferred. A company car may be provided in select markets.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record

Responsibilities

• Provide advanced primary care to high-acuity patients in the home setting.
• Address patient needs with time and attention, focusing on quality care rather than volume.
• Collaborate with care teams to support value-based in-home primary care initiatives.
