Job summary

Monogram Health is seeking an experienced Float Advanced Practice Provider (NP/PA) to deliver in-home care for patients with multiple chronic conditions in Texas, including Austin, New Braunfels, and San Antonio. You’ll work with a multidisciplinary team and provide care in the home and via telehealth.

Responsibilities include assessments, wellness visits, care education, and coordination with PCPs, with a structured schedule and a strong emphasis on value-based care.

Qualifications

• Active and unrestricted RN and NP license, or PA license.
• Board certification (NP or PA).
• Current and unrestricted DEA certificate, or eligibility to obtain.
• Minimum of two years direct patient care experience.
• Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance.
• BLS certification on onboarding.
• Knowledge of federal/state regulations (HIPAA/OSHA).
• Experience with EHR systems (Athena, Salesforce, Epic).
• Experience with annual wellness or comprehensive visits preferred.

Responsibilities

• Provide evidence-based, patient-focused home care to reduce hospitalizations.
• Perform comprehensive assessments and wellness exams.
• Educate patients on disease management and medications.
• Participate in virtual care team meetings.
• Engage in quality assurance and documentation reviews.
• May practice in SNFs, ALFs, hospitals, and other settings.
• Coordinate care with PCPs, specialists, and services.
• Complete documentation accurately and timely.
