Youth Villages seeks a PRN Community-Based Paraprofessional to support the Intercept program, delivering in-home services that help keep families together. You will coordinate transports, supervise visits, and document activities for ongoing care coordination in the family’s home and community settings.

Responsibilities include teaching parenting skills, providing routine reports, and maintaining strong communication with referral sources.

Competenze

• High School diploma or equivalent (required).
• Experience with at-risk youth and/or families preferred.
• Strong organizational skills with attention to detail.
• Excellent written, verbal, and oral communication skills.
• Ability to manage multiple priorities simultaneously.
• Basic computer knowledge and familiarity with EMR systems.
• Flexible schedule required to align with youth, adults and families served.

Mansioni

• Coordinate transports and supervised visits and maintain contract-required documentation.
• Teach and demonstrate parenting skills.
• Provide daily and weekly reports to referral sources and direct supervisor.
• Maintain professional communication with referral sources.
• Complete ongoing assessments as outlined by the referral source.
• Attend weekly meetings for professional development and booster trainings.
• Document accurately in the EMR system.
• Perform other duties as assigned.
