Qualifications

• Bachelor’s degree in any human services or education field is required.
• Associate’s degree with 2 years of related experience and pursuing a bachelor’s degree within 6 months is acceptable.

Responsibilities

• Provides in-home services per MFT contract requirements.
• Delivers up to 180 days (270 days with extension) in-home services.
• Maintains face-to-face contact per contract-defined frequency and client needs.
• Completes required MDHHS and MFT documentation on time.
• Engages families using Motivational Interviewing to assess strengths, needs, culture and values.
• Provides access to housing, transportation, employment, education, benefits and other supports.
• Drives agency business and travels as required.
• Participates in training and meetings as required.
• Submits to background checks and adheres to agency policies.
