Catholic Charities West Michigan (CCWM) is seeking a MiFamily Together Advocate to deliver in-home services within Kent County under the MFT framework. The role focuses on family support, safety planning, and coordinated care with MDHHS and partner agencies.

The position requires a valid driver’s license, reliable transportation, and flexible scheduling to meet client needs. Wage is up to $47,715.20 annually based on experience.

Qualifications

• Bachelor’s degree in human services or education required or equivalent.
• Experience working with families and children preferred.
• Ability to work in home-based settings and coordinate with MDHHS/agency partners.

Responsabilités

• Provides in-home services under the MFT contract.
• Delivers services for up to 180 days with approved extensions to 270 days.
• Maintains face-to-face contact with families as required by the contract.
• Completes MDHHS/MFT documentation in a timely manner.
• Uses Motivational Interviewing to build rapport and assess needs and goals.
• Provides access to concrete supports and resources (housing, transportation, education, benefits).
• Develops safety and prevention plans and coordinates with partners and case managers.
• Maintains flexible scheduling to meet client needs and crisis interventions.
