Job summary

Intermountain Centers in Phoenix, AZ is seeking a Family Support Specialist to work with families, providing resources, advocacy, and guidance to help navigate parenting, education, healthcare, and social services.

You will meet clients in home and community, develop person-centered support, and collaborate with the Child and Family Team to create individualized plans and ensure outcomes. Driving is required to meet clients and provide in-home support, with crisis intervention as needed.

Qualifications

• High school diploma or GED with 4 years of experience working with children with a mental diagnosis.
• Associate’s degree with 2 years of experience working with children with a mental health diagnosis.
• Bachelor’s degree or higher with 1 or more years of experience working with children with a mental health diagnosis.
• Bachelor’s degree in behavioral health related field (i.e., Psychology, family studies, human development, sociology, etc.) Some experience with mental health preferred.

Responsibilities

• Provide support to enrolled children and families as outlined by the Child and Family Team service plan.
• Develop and implement individualized behavior intervention plans and support plans based on assessments, data analysis and goals.
• Assist children and families by identifying strengths and needs, accessing support, and collaborating with other departments and agencies.
• Driving is required to meet clients where they are in the community and adhere to client’s availability hours.
• Coordinate with social workers, counselors and other professionals to coordinate comprehensive care.
• Provide crisis intervention and emotional support to families.
