Job summary

Maple City Health Care Center is seeking a Family Support Specialist to partner with families, provide in-home case management, and navigate community resources. You will support parent education and culturally competent services in Elkhart County, with emphasis on trauma-informed care.

The role requires strong English communication, ability to work with diverse families, and occasional travel for in-home visits.

Qualifications

• Must have knowledge of child development and family dynamics.
• Strong written and verbal communication in English; Spanish preferred.
• Ability to manage time, organize caseloads, and document thoroughly.

Responsibilities

• Articulate materials and ideas with families in an easy-to-understand manner.
• Work with staff and referral sources to serve families based on needs.
• Ensure all program documentation is complete and timely.
• Maintain electronic case files according to policy.
