Job summary

New Mexico Children, Youth and Families Department (CYFD) seeks an In-Home Services Practitioner to develop and implement child and family interventions, focusing on safety and well-being. You will assess risk, coordinate services with families, and ensure proper documentation and case management.

The role involves collaborating with community partners, planning services, and guiding families through safety plans and resources to support child welfare goals.

Qualifications

• In-depth assessment of safety issues and protective factors for children receiving in-home services.
• Development, implementation, and monitoring of in-home services plans.
• Determine appropriate contact level with families to promote safety and monitor plans.
• Conduct safety assessments prior to case closure and address outstanding issues.
• Utilize safety assessment tools to guide process and documentation.
• Identify community resources and make referrals with follow-up.

Responsibilities

• Provide In-Home Services for children and youth to promote safety and well-being.
• Conduct ongoing case activities; collect information and create plans.
• Provide case management; document efforts in case record system.
• Collaborate with internal and external partners to ensure service quality.

Education

Master's Degree in Social Work, Guidance and Counseling, Counseling, Psychology, Sociology, or Criminology

Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, or Family Studies
