Job summary

New Mexico CYFD seeks an In-Home Services Practitioner to develop and implement interventions for children and families receiving non-legal services. You will assess safety, create and monitor case plans, coordinate with caregivers and community partners to ensure safety and well-being.

Qualifications

Why does the job exist?

In-Home Services Practitioner workers develop and implement interventions with children and families receiving services to enhance families capacity to provide stability for their children and enhance child well-being. They work collaboratively with the children, families, caregivers, and internal and external partners to ensure the provision of quality services so as to achieve the goals of safety and well-being for children and families served by the Children Youth and Families Department (CYFD). They will assess the risk and safety of the child and take appropriate action including referring parents and caregivers to safety related services, safety planning with families, and/or preparing cases for transfer to Community Bases Services. They make day-to-day decisions regarding families in crisis, identify services/resources needed to address risk and safety issues where children are at risk of placement in substitute care and are subject to treatment plans determined by CYFD workers.

How does it get done?

Provide In-Home Services for children and youth served by the agency to reduce risk and promote the safety and well-being of children and youth receiving non-legal services (30%).

Conduct ongoing case activities to collect information, create appropriate plans, reduce risk, promote safety and ensure well-being for children and youth receiving non-legal services in order to enhance families' capacities to provide for their children and youth (30%).

Provide case management services for families served by the agency to include but not limited to documenting efforts and case activities in case record system and ensure all required paperwork is completed (20%).

Provide quality In Home Services to children, youth and families served by the agency by working collaboratively with internal and external partners to ensure the quality of services to achieve the goals of safety and well-being for the children, youth and families served through In Home Services (20%).

Ideal Candidate

Experience as it relates to:

• Conduct assessments to identify safety issues and protective factors for children receiving in-home services.
• Develop, implement, and monitor in-home services plans.
• Determine the amount of contact that the family needs to promote safety and effectively monitor the in-home services plan and provide contact at that level.
• Conduct safety assessments prior to case closure and address outstanding safety issues.
• Utilize the New Mexico Safety Assessment tool to guide, process, and document the assessment and plan.
• Review SDM Risk and Family Needs and Strength instruments as part of process to identify family concerns.
• Work with family to identify additional needs, including but not limited to medical, educational, dental, and mental health needs of children.
• Create a written case plan outlining the intervention and the desired results.
• Identify community resources that are available and adequate to meet the needs of the family.
• Make referrals to community providers/resources as appropriate and provide follow-up to ensure family has the opportunity to connect with providers.
• Teach family members skills to enhance the family's capacity to care for their children.
• Work with community providers to assess family progress.
• Collaborate across service areas and with providers to ensure comprehensive services for families in need.
• Document case plans (safety/assessment & treatment) and activities in case record system.
• Secure releases so as to communicate with providers and other natural helpers involved with the family.
• Document the need for and the purpose of any emergency discretionary fund expenditures.
• Coordinate and/or participate in IPM, and/or multidisciplinary meetings with families to develop appropriate case plans.
• Partner with community providers and other Department services to assist families in enhancing their capacity to safely and appropriately care for their children.

Master's Degree in Social Work, Guidance and Counseling, Counseling, Psychology, Sociology, or Criminology from an accredited college/university OR Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, or Family Studies from an accredited college/university and two (2) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health, and/or health care. Any combination of education from an accredited college or university in a related field above licensure requirements and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience. May require a current license in New Mexico as a Doctoral Level Psychologist, Psychologist Associate, LISW, LMSW, LPC, LPCC, LMHC, or LMFT or eligibility for such licensure in accordance with New Mexico requirements.

Employment Requirements

Pre-employment background investigation is required and is conditional pending results.

Working Conditions

Work is performed in an office setting with extensive exposure to regular periods of video display terminal and keyboard usage and phone usage. Travel may be required. Possible exposure to irate clientele. Incumbent will work under stress and frequent time constraints. The Statewide Central Intake operates 24 hours a day, 7 days a week.
