Job summary

New Mexico CYFD seeks an In-Home Services Practitioner to develop and implement interventions for children and families receiving non-legal services. You will assess safety, create and monitor case plans, coordinate with caregivers and community partners to ensure safety and well-being.

Responsibilities include conducting risk and safety assessments, developing service plans, referring to medical, educational, dental, and mental health resources, and documenting progress in the case records.

Qualifications

• Master's Degree in Social Work or related field; two years of health/social work experience may substitute for education
• Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, or Family Studies plus two years of relevant experience
• Current NM social work license or eligibility is preferred

Responsibilities

• Provide In-Home Services to enhance safety and well-being for children and families (non-legal services).
• Conduct ongoing case activities to collect information and reduce risk through well-being plans.
• Prepare and document case plans and coordinate with community providers for needed services.
