Job summary

Health Connect America, Inc. is seeking a Family Centered Treatment (FCT) Specialist. You will deliver evidence-based services in home and community settings, coordinating care with the family’s Treatment Team and partners to ensure holistic support.

The role requires NC QP credentials, a relevant degree, a valid driver’s license, and full-time supervision experience with children/youth with mental health needs. Join a mission-driven network committed to trauma-informed care.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104. One of the following:
• Master's in human services field + 1 year experience
• Bachelor's in human services field + 2 years experience
• Bachelor's in non-human services field + 4 years experience
• Current NC license (including provisional) or board‑issued certification in a human services profession
• Experience must be full‑time and supervised, serving children and adolescents with a primary mental health diagnosis, and may be accumulated before or after degree completion.
• Must have a valid driver's license and access to a fully insured, private form of transportation

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model.
• Serve as a central point of coordination for each family's Treatment Team.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record.
• Participate in Quality Assurance activities.
