Job summary

Health Connect America is seeking a Family Centered Treatment Specialist to deliver client-centered psychotherapy to children, adolescents, and families using the FCT model. The role emphasizes home- and community-based service delivery with flexible scheduling to meet family needs.

You will work with a Treatment Team to develop and monitor Service Plans, conduct progress reviews, and provide psychoeducation and family-focused interventions. Strong documentation and collaboration are essential.

Qualifications

Bachelor's degree in human services or related field.
Two years of experience working with youth or families.
Current Family Centered Treatment Certification preferred.
Experience delivering services in home or community settings.

Responsibilities

Provide Family Centered Treatment (FCT) services per model fidelity requirements.
Complete FCT checkoffs and trainings within deadlines.
Deliver FCT services to children, adolescents and families in home/community settings.
Provide 24/7 on-call coverage for a caseload of families and children.
Complete clinical records and FCT documents in the EHR promptly.
Develop and coordinate Service Plans with the Treatment Team.
Conduct Service Plan revisions and implementation.
Provide psychoeducation as indicated in the plan.
Coordinate discharge planning and linkages to aftercare resources.
Coordinate initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan reviews.
Consult with medical and non-medical providers and engage supports.
