Job summary

Pinnacle Family Services seeks a Family Centered Treatment (FCT) Specialist to provide evidence-based services in homes and communities. You will collaborate with families and a multidisciplinary Treatment Team to support trauma-informed care and sustainable well-being.

The role requires NC QP credentials, full-time supervised experience with children and adolescents with mental health needs, and a valid driver’s license. Join a mission-driven network backed by Health Connect America.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver's license and access to a fully insured, private form of transportation.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model to set goals and build skills.
• Coordinate with medical, behavioral health, school, and community partners for seamless care.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain timely documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to improve programs.
