Job summary

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities, guiding families through the FCT model and coordinating with a multi-disciplinary team.

The role requires NC QP credentials, licensure or certification, and a full-time commitment with on-site community work. A valid driver's license and transportation are required.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Experience must be full‑time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver's license and access to a fully insured, private form of transportation.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, helping them set goals and build skills.
• Serve as a central point of coordination for each family’s Treatment Team, collaborating with medical, behavioral health, school, and community partners.
• Develop, implement, and refine individualized Service Plans with families you serve.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely, accurate documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to strengthen programs.
