Job summary

Ramey-Estep Homes, Inc. seeks a Case Coordinator for the KSTEP program to provide in-home, intensive case management for up to 9 families.

The role includes assessments, implementing evidence-based treatment, referrals, and direct therapeutic support for up to 8 months per family, with ongoing data collection. You will report weekly to the DCBS worker, maintain Credible notes and KSTEP data, coordinate family team meetings, develop Action and Treatment Plans, and ensure discharge and aftercare

Qualifications

• Bachelor’s degree in a human services field from an accredited school.
• Experience working with multi-problem families; crisis intervention familiarity.
• Ability to develop specific goals and teach needed skills.

Responsibilities

• Maintains a caseload of up to 9 families.
• Enters case notes into Credible in a timely manner with concise summaries.
• Maintains up-to-date information in the KSTEP database.
• Meets all timeframes for case contacts with families, providers, and DCBS workers.
• Provides weekly activity summaries to DCBS workers.
• Completes required modalities with fidelity (NCFAS, Motivational Interviewing, CARES, Solution-Based Casework).
• Schedules and attends all family team meetings and offers progress recommendations.
• Ensures timely Action Plans and Treatment Plans focused on safety and functioning per the SBC plan.
• Ensures confidentiality and compliant information sharing.
• Participates in on-call schedule.
