Job summary

Ramey-Estep / Re-group is seeking a Case Coordinator to deliver intensive in-home case management under the Kentucky Strengthening Ties and Empowering Parents (KSTEP) program.

You will manage up to 9 families, conduct assessments, and implement evidence-based treatments while coordinating with DCBS and community resources to sustain safety and functioning for up to 8 months per family.

Qualifications

• This role requires a Bachelor’s degree in a human services field and experience with multi-problem families.
• Experience in crisis interventions and ability to set behavioral goals is preferred.
• Proficiency with evidence-based modalities and coordination with DCBS is expected.

Responsibilities

• Maintain caseload of up to 9 families and ensure timely case notes in Credible.
• Complete accurate data entry in the KSTEP database and provide weekly DCBS progress reports.
• Facilitate family team meetings and develop Action and Treatment Plans driving safety and recovery.
• Coordinate referrals, discharge planning, and aftercare per program guidelines.
• Participate in on-call rotations and community networking activities.
