Job summary

KVC Kentucky is seeking a Family Care Coordinator/Case Manager/Social Worker to work directly with families in Louisville, KY. The role involves in-home services, safety planning, and coordination with DCBS and providers to support child safety and family well-being.

You will engage with families to implement action plans, facilitate communication among caregivers, and maintain ongoing support across services while adapting to a flexible schedule and regional travel as needed.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Ability to provide in-home services to children and families.
• Experience working with complex and multi-need families.

Responsibilities

• Establish initial contact with families within 24 hours of referral to assess safety and plan next steps.
• Coordinate care by connecting providers with the family and maintaining ongoing communication.
• Facilitate the Family Action Plan by coordinating with all identified providers and monitoring progress toward goals.
• Maintain flexible work hours including nights/weekends and travel regionally; participate in on-call rotation.
