Qualifications

• Experience with the Juvenile Justice systems.
• Experience or knowledge of Foster Care, Adoption Assistance, and Behavioral Health services in Kentucky SKY populations.
• 2 years’ experience in behavioral health, social services or juvenile justice.
• Computer literacy and proficiency in MS Office (Word, Excel).

Responsibilities

• Evaluate member needs using care management assessments and data review to propose health care plans.
• Identify high-risk factors and service needs impacting outcomes and intervene early.
• Coordinate Wellness care plan activities and monitor member care needs.
• Ensure quality of care via regulatory and internal guidelines and collaborate with Medical Directors whenever needed.
• Collaborate with case managers and other health programs to overcome barriers to goals and objectives.
• Assist members in engaging with providers to support informed health decisions.
