Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field.
• Completion of state-required case management/service coordination course within six months of employment.

Responsibilities

• Conducts and documents comprehensive assessment of client needs.
• Participates in development of client's service plan.
• Arranges for delivery of needed services identified in assessment.
• Assists client in accessing needed services via referrals or linkages.
• Monitors client's progress with referrals and follow-ups; reassess needs.
• Performs crisis intervention and coordinates emergency services when needed.
• Maintains case records per SCS and regulatory standards.
• Provides case consultation with service providers.
• Communicates with supervisor about case management issues and resources.
• Provides psychosocial education to clients and/or family members.
• May serve as Primary Case Coordinator (PCC) depending on skill and autonomy.
