CVS Health is hiring a Juvenile Justice Care Management Coordinator to support members in Kentucky across the Jefferson Region. This field-based role requires 80–90% in-state travel and travel to Louisville for meetings and training.

A bachelor’s degree or non-licensed master level clinician in behavioral health or related field is required, with experience in juvenile justice and related services. You will perform care management assessments, coordinate wellness plans, and engage with members

Qualifikationen

• Experience with Juvenile Justice systems and related services is required.
• Knowledge of Foster Care, Adoption Assistance and Behavioral Health services preferred.
• Experience with trauma-informed care and crisis intervention is desirable.

• Evaluate member needs and coordinate care to improve health outcomes.
• Collaborate with care managers, supervisors and Medical Directors to address barriers to care.
• Document care activities and monitor quality of care in line with regulatory requirements.
