Job summary

Wise & Healthy Aging in Santa Monica is looking for a Care Coordinator to provide comprehensive care management services to older adults and disabled residents. Qualifications

• Minimum of two years’ experience in gerontology, long-term care, or social work.
• Ability to communicate effectively in writing and orally.
• Valid driver's license and auto insurance required.

Responsibilities

• Conduct biopsychosocial in-home assessments for clients.
• Develop a written care plan with measurable outcomes.
• Arrange and coordinate necessary services for clients.
