Job summary

Wise & Healthy Aging seeks a full-time Elder Abuse Advocate for the HEART program, based in the Van Nuys area. The role provides field-based case management, counseling, and victim advocacy to elders and dependent adults, with duties across intersectional care teams in LA County.

This position offers a mix of field visits and office work, with professional development in aging-related interventions. Responsibilities include home visits, care planning, crisis safety, and collaboration with APS,

Qualifications

• Bachelor’s degree in social work, or related field.
• MSW preferred.
• Two years in social work, gerontology, or related fields.
• Experience with victims and aging adults.
• Bilingual in Spanish preferred.

Responsibilities

• Provide field-based geriatric case management and counseling to elder abuse survivors.
• Respond to referrals from Adult Protective Services, police, and other sources.
• Conduct home visits and develop care plans with clients.
• Coordinate safety planning and linkages to essential services.
• Assist with paperwork for restraining orders and court hearings.
• Offer outreach and education about elder abuse in the community.
• Participate in weekly multidisciplinary team meetings with professionals.
