Job summary

Herewith is seeking compassionate In-Home Dialysis Care Partners to support patients receiving dialysis in the comfort of their homes in Thousand Oaks, CA. You will work with patients, ensuring safe environments, proper pre-treatment prep, and ongoing wellbeing monitoring.

Qualified candidates have at least 1 year of dialysis machine experience, strong interpersonal skills, and the ability to work independently while staying connected with a clinical care team.

Qualifications

• Care Partners with at least 1 year of dialysis machine experience.
• Prior experience supporting dialysis patients (home or clinical).
• Familiarity with home dialysis machines is preferred.
• Reliable, empathetic, and detail-oriented.

Responsibilities

• Conduct and support patient setup for dialysis treatments (NxStage, Tablo or equivalent).
• Provide companionship, emotional reassurance, and schedule adherence.
• Monitor safety and assist with light activities of daily living (ADLs).
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during support hours.
