Job summary

Providence at Home with Compassus is seeking a Certified Occupational Therapy Assistant to deliver home-based therapy under physician orders and an OT/OTL plan of care. You will work within a multidisciplinary team, communicating patient status and coordinating care to maximize outcomes in the home environment.

The role emphasizes accurate documentation, timely progress notes, and discharge planning for diverse ages from neonate to geriatric.

Qualifications

• Education to meet certification, license or registration requirements.
• Degree from an accredited Occupational Therapy Assistant Program.
• Associate’s Degree Occupational Therapy from an accredited COTA program.
• Graduate of an accredited school of Certified Occupational Therapist Assistant.

Responsibilities

• Provides Occupational Therapy services under the direction of an OT/OTL per Plan of Care.
• Communicates with multidisciplinary team to form a collaborative client care plan.
• Documents treatment and coordination of care contacts per agency guidelines.
• Plans for discharge and participates in reassessments and discharge planning.
• Educates staff and caregivers about therapy services in the home setting.
• Maintains timely documentation and adheres to productivity standards.
