Qualifications

• Valid driver’s license is required.
• Experience or prior training with mental health/substance use population preferred.
• Knowledge of community resources preferred.
• Ability to learn and use the EHR system daily.
• Strong interpersonal skills and confidentiality handling.
• Clear written and verbal communication with good organization.
• Creativity and problem-solving in challenging situations.
• Able to lift up to 25 lbs.

Responsibilities

• Oversee consumers in daily activities in community/home/settings.
• Assist with self-care, independence, mobility, and learning.
• Plan meals, shop, and help with grocery shopping as needed.
• Organize leisure activities and guide appropriate behavior.
• Accompany consumers to appointments and community activities.
• May monitor or administer medications as AMAP certified.
• Document vital signs and observations per policy.
• Maintain confidentiality and HIPAA compliance.
• Utilize technology to facilitate services and training.
• Meet CPR/First Aid and crisis intervention training requirements.
