Job summary

Prestera Health Services in Huntington, WV is seeking a Direct Service Provider to supervise and assist consumers in community, home, or treatment settings. You will support daily living, meal preparation, and coordinated activities while promoting safety and well-being.

The role requires a High School Diploma or GED, a valid driver’s license, and CPR/First Aid training. Experience with mental health and substance abuse populations is preferred; knowledge of community resources is beneficial.

Qualifications

• Valid driver's license required.
• Experience or prior training working with mental health and substance abuse population preferred.
• Knowledge of community resources preferred.
• Ability to learn, navigate, and work daily in the EHR system effectively.
• Excellent interpersonal skills and ability to develop trusting relationships and handle sensitive situations.
• Good written and verbal communication skills with time management and organizational skills.
• Creative and solution-oriented mindset.
• Must be able to lift to 25 lbs.
• CPR-First Aid and crisis intervention training required.

Responsibilities

• Assists consumers in self-care, self-direction, independence, mobility, and learning per service plan.
• Performs light housekeeping, plans meals, and grocery shopping assistance as needed.
• Organizes/leads leisure activities and guides appropriate behavior.
• Accompanies consumers to medical appointments, errands, and meetings.
• May monitor or administer medications as AMAP certified.
• Provides education or supportive services to consumers.
• Documents vital signs and consumer observations per policy.
• Monitors behavior and addresses issues per treatment plan.
• Ensures consumer safety and directs emergencies during shift.
• Uses effective communication and de-escalation skills.
• Contributes to crisis intervention as trained.
• Transports consumers in Prestera or personal vehicle as needed.
• Adheres to policies, confidentiality, HIPAA, and incident reporting.
• Utilizes technology to facilitate services and improve efficiency.
• Meets training requirements including CPR/First Aid.
