Prestera Center for Mental Health Services, Inc. is seeking a Direct Service Provider to work in Pinecrest/Huntington area, providing oversight and daily support in community, home or residential treatment settings.

The role involves assisting with self-care, independence, and learning, following each consumer's service plan. This position requires a high school diploma, CPR/First Aid, a valid driver's license, and the ability to document observations, de-escalate when needed, and transport

Competenze

High School Diploma or GED required.
Maintains First Aid/CPR certification.
Crisis prevention/intervention training completed or maintained.

Mansioni

Assist consumers with self-care, independence, mobility and learning per service plan.
Perform light housekeeping and meal planning/prep when assigned.
Organize and participate in leisure activities and guide appropriate behavior.
Accompany consumers to medical appointments, grocery shopping, and meetings.
Monitor self-administration of medications or provide AMAP medication administration if certified.
Document vital signs and consumer observations per policy.
Monitor behavior and address issues per treatment plan; coordinate with leadership.
Ensure safety and manage emergencies during shift.
Communicate effectively and use de-escalation techniques.
Transport consumers as needed in Prestera or personal vehicles.
