Job summary

Future Transitions is seeking staff to provide in-home and community services for individuals with Developmental Disabilities in California. The role involves teaching daily living skills, following a service plan, and working in diverse community settings to support independence.

Qualifications include experience with Developmental Disabilities, CPR/First Aid certification (or within 30 days of hire), and strong communication and problem-solving abilities.

Qualifications

• Experience working with persons with Developmental Disabilities.
• CPR/First Aid certification for adults and children (or to be completed within 30 days of hire).
• Minimum one year experience preferred or education in the field working with persons with Developmental Disabilities.
• Ability to communicate with Consumers and Supervisors, problem solving ability.
• Must be 18+ years of age.

Responsibilities

• Train or teach individuals with Developmental Disabilities to accomplish daily living skills in home and community.
• Follow Individual Service Plan and work basic-level cases; provide training in listed areas.
• Locations include client homes, schools, community settings, and various venues (shopping, banks, DMV, etc.).
• Locate resources, complete hours, maintain records, and report as needed; report abuse/neglect when observed.
