Job summary

Future Transitions in Chatsworth, CA is seeking a part-time to full-time Social Services role to provide in-home and community support for individuals with Developmental Disabilities. The position emphasizes instruction in daily living skills and adherence to an Individual Service Plan.

A driver’s license, car, and background check are required, with CPR/First Aid certification preferred or to be completed within 30 days of hire.

Qualifications

• Must have experience working with persons with Developmental Disabilities.
• Ability to communicate with Consumers and Supervisors, ability to problem solve.
• CPR/First Aid certification for adults and children (or to be completed within 30 days of hire).
• Minimum one year experience preferred or education in the field working with persons with Developmental Disabilities.

Responsibilities

• Train or teach children and/or adults with Developmental Disabilities to accomplish daily living skills in the home and in the community.
• Follow Individual Service Plan and work basic-level cases.
• Provide training in areas such as toilet training, hygiene and grooming, travel training, and social skills development.
• Locate community resources, complete hours, maintain records, and submit reports.
• Identify and report possible abuse/neglect.
